Ultimate Coders — Participation Agreement, Release and Consent (In-Person Programs) I am the parent or legal guardian of the child named in this registration, and I sign this agreement for myself and on the child's behalf. 1. Activities. My child will take part in coding, robotics, electronics, 3D design and printing, and related activities at an Ultimate Coders centre, including camps, events, parties and occasional supervised activities on the centre's premises. Activities may involve computers, tablets, robotics kits, small parts, batteries, hand tools, 3D printers and other equipment. 2. Risks. I understand that, like any activity with other children and equipment, these activities carry some risk of injury, including trips and falls, minor cuts or burns from equipment, choking on small parts for young children, eye strain, allergic reactions, and injury caused by other participants. I accept these ordinary risks for my child. 3. Rules and conduct. My child will follow staff instructions and centre rules, use equipment only as shown, and treat others with respect. Ultimate Coders may remove a child from an activity, without refund for that session, if their behaviour puts themselves or others at risk. 4. Health information and first aid. The health details I give (allergies, medical conditions, medication, emergency contact) are true and complete, and I will update them when they change. If my child is hurt or unwell, I authorize staff to give first aid and, if they think it necessary, to call emergency services and arrange medical treatment. I will pay any resulting costs not covered by insurance. 5. Pickup. My child will be picked up on time by me or by a person on the pickup list in this registration. Staff may ask for identification and will not release my child to anyone else. If I have said my child may sign out on their own, I accept responsibility for them once they leave the centre. 6. Photos and work. Ultimate Coders may photograph or record class activities and my child's projects for teaching, for progress updates to families, and for its website and social media, without showing my child's full name. I can withdraw this consent for promotional use at any time by telling the centre. 7. Personal items. Ultimate Coders is not responsible for loss of or damage to personal devices or belongings brought to the centre. 8. Release. To the fullest extent permitted by law, I release Ultimate Coders Franchising Inc., the franchisee operating this centre, and their owners, staff and volunteers from claims I may have for injury, loss or damage arising from my child's participation, except where caused by their gross negligence or wilful misconduct. Nothing in this agreement limits any right my child has under the law of their province. 9. Agreement. I have read this agreement, I understand it, and I sign it voluntarily. It applies to every program my child attends at Ultimate Coders until I am asked to sign a new version.
